ACH Authorization for Employees

ACH Direct Deposit for Employees ONLY
Vendors interested in ACH should contact Accounts Payable, glenda.king@uconn.edu

Following is the criteria for this service:

  • Offered to Faculty & Staff of the University of Connecticut ONLY
    Graduate Assistants or Students are not eligible.
  • Payments will be deposited to your CHECKING account.
  • Your e-mail address is essential. This is used to inform you of any deposits.
  • Account must be with a United States bank, no foreign bank accounts.
  • The form must be completed in its entirety.
  • Attach a copy of a void check for verification purposes.

To make changes to your account, please complete the ACH AUTHORIZATION REVISION FORM

 

Important Information :

If you close your account you must notify the Accounts Payable Department immediately , either by phone at (860) 486-4137 or via email to glenda.king@uconn.edu
